We can complete your child’s sports physical and have the form ready for you before leaving the office. Parents are asked to complete the health history portion before the visit with the provider. Please bring a copy of the patient’s vaccination record with you.

The school system requires sports physicals completed April 15 or after for that school year. Please feel free to print out the history part of the form prior to coming to the office for the sports physical.
