You can complete the forms to the right prior to arrival to save time when you get to the office. If you are unable to print out or complete it online, we can give you paperwork to complete when you arrive. You will want to arrive about 15-20 minutes per child you are registering as a new patient to complete paperwork in the office.

• Download and Fill out the forms prior to your first visit.
• This will save you time when you visit the clinic for the first time.
• If you choose to fill out the forms in the clinic you must come 20 minutes early (for each new patient!)
• If you do not allow enough time to fill out the forms OR bring them with you completed, your appointment may be rescheduled.
